What is a Community Health Centre: A Community Health Centre (CHC) delivers health care in a way that takes the whole person and their community into consideration. The programs and services offered by a CHC are designed in direct response to the unique and ever changing needs of the communities they serve. Everyone matters at a CHC - we have something for everyone!


What we do: Our Bridges CHC team includes physicians, nurse practitioners, nurses, dietitians, diabetes educators, social workers, health promoters, community health workers, a physiotherapist, kinesiologist and physiotherapist assistant. To promote healthy living in our communities, our programs and peer support groups can help you with smoking cessation, diabetes education, mental health, chronic pain management, stress management, exercise and healthy eating. 

Who we serve: Bridges Community Health Centre serves the communities of Fort Erie, Port Colborne and Wainfleet with a focus on the needs of seniors, children and youth, newcomers to Canada, and families or individuals with low incomes. For those without a family doctor, our Primary Care team is here to support you. For those who are looking to improve their health and wellbeing, our educational and practical health promotion programs and services are available at no cost and open to all members of the community.


Funding Sources: Bridges CHC receives funding from the Hamilton Niagara Haldimand Brant Local Health Integration Network (HNHBLHIN) and the Ministry of Health and Long-Term Care (MoHLTC).
