    HEALTH PROMOTION AT BRIDGES CHC

     

    Bridges CHC Health Promotion team consists of two Health Promoters and two Community Health Workers.  

     

    HEALTH PROMOTERS

    Health Promotion, as defined by the Ottawa Charter (1996), is "the process of enabling people to increase control over, and to improve, their health”. While health promotion embraces actions directed at strengthening the skills and capabilities of individuals, it is also a comprehensive social and political process where actions are directed towards changing social, environmental and economic conditions so as to alleviate their impact on public and individual health. The process of enabling people to increase control over the determinants of health ultimately improves their health.

     

    Health Promotion strategies are supported by five priority action areas as outlined in the Ottawa Charter for health promotion:

     

    • Build healthy public policy

    • Create supportive environments for health

    • Strengthen community action for health

    • Develop personal skills, and

    • Re-orient health services

     

    COMMUNITY HEALTH WORKER

    Feeling stressed out, overwhelmed or isolated?  A Community Health Worker can help by:  

     

    • Offering non-judgemental, confidential support

    • Informing you of available services and resources in our community

    • Referring you to services and advocating that you receive the help you need

    • Assisting you with form filling and attending appointments

    • Offering volunteer opportunities within our Community Health Centre and informing you of other ways to get involved and make a difference
