Diabetes Education 

The goal of Bridges CHC's Diabetes Education Program is to encourage self-management of diabetes. Our diabetes educators meet with clients one-on-one and offer group counselling sessions. The topics covered can help you reduce the risk of and manage your diabetes while connecting you to people living with the same condition. Along the way you will have the help of a diabetes team who can answer any questions you may have. Best of all, it’s free to attend. You will learn about topics like:

The Diabetes Education Program at Bridges CHC is hosted at both sites in Fort Erie and Port Colborne/Wainfleet and open to anyone who lives in these communities. A team of diabetes nurse educators and registered dietitians work with health care providers (physicians, nurse practitioners, nurses, pharmacists, podiatrists and social workers) to provide the best care for people living with or at risk of diabetes.
